Enroll the UNDERSIGNED to accept patients at a discounted rate (as indicated below) when said patient(s) presents a bona fide Student Lifeline Card along with immediate payment at time of office visit/treatment. This discount is only applied to office visits/treatments and excludes any and all other medical services (non-hospital). In consideration of discount offer by physician/medical service provider and an annual registration fee ($39 for Sole Practitioner, $79 for Multiple Medical Service Facility), Student Lifeline, Inc. shall direct all student inquiries among students (ages 15-21) or if a pediatric provider (Pre-K to 21), to stated UNDERSIGNED and further broadcast UNDERSIGNED to all local area schools, PTAs, colleges, and youth organizations. Upon receipt of this form, we shall contact you further in in order to finalize Agreement (Payment, etc.).
